摘要

目的:分析急诊科动脉夹层（Aortic dissection

AD）患者的临床特征及预后

为急诊科诊治该病提供依据与预防对策。方法:回顾分析6年期间50例AD患者中24例急诊患者的临床特征

误诊率及预后。结果:急诊者占48%（24/50）

其中87.5%（21/24）患者有疼痛症状

发病时有高血压者占66.7%（16/24）

急诊误诊率50%（12/24）

死亡率33.3%（8/24）。结论:AD患者中高血压的患病率较高;男性多于女性。疼痛是急诊AD的主要临床表现

疼痛部位与AD发生部位密切相关

当同时出现血压不对称、休克、纵隔增宽时

高度提示预后不良。超声

CT和MRI是诊断AD的很好技术措施。提高对AD的认识

加强多学科合作及医患沟通

才能有效降低该病的风险。

Abstract

Objectives:To analyze the clinical manifestations of acute AD so as to improve diagnosis and treatment.Method:50 cases of AD were studied retrospectively.Results:24 out of 50 cases were acute AD.Among them

21patients(87.5%) suffered pain

16 cases(66.7%) with hypertension.The misdiagnosis rate was 50% and mortality rate was 33.3%(8/24).Conclusions:(1)There was a high rate of hypertension among AD patients and it occurs more among male than female.(2)Pain is the main clinical manifestation and the affect place was closely related to the AD location.When there is asymmetrical blood pressure

shock and wider mediastinum

the prognosis is severely negative.(3)Ultrasound

CT and MRI are good examination techniques for AD(4)Attetion on AD

multidisciplinary cooperation

and doctor-patient communication can reduce the risk of this disease.
