Objective To explore the effect of out-of-hospital tracking management combined with nutrition and health guidance in the continuous care of patients with chronic obstructive pulmonary disease(COPD) complicated with drug-resistant pulmonary tuberculosis
so as to provide a reference for clinical treatment. Methods A total of 56 COPD patients with drug-resistant pulmonary tuberculosis admitted to the Third People’s Hospital of Shantou from January 2016 to January 2020 were selected as the study subjects. The patients were randomly assigned to the control group and observation group
with 28 cases in each group. The patients in the control group were given routine continuous care
and the patients in the observation group were given continuous care of out-of-hospital tracking management combined with nutritional health guidance on the basis of routine continuous care in the control group. The St. George’s Respiratory Questionnaire(SGRQ) from St. George’s Hospital was used to assess the quality of life at discharge and 3 months after discharge in the 2 groups. Forced expiratory volume in 1 second(FEV
1
) and forced vital capacity(FVC) levels were compared between the two groups at discharge and 3 months after discharge. The COPD assessment test(CAT) was used to assess the effect of COPD on quality of life at discharge and 3 months after discharge in the 2 groups. Results At discharge
there was no significant difference in clinical symptoms
activity ability
and disease impact score in the SGRQ between the two groups(P
>
0.05).At 3 months after discharge
the clinical symptoms
activity ability
and disease impact scores in the SGRQ of the patients in the 2 groups were lower than those at discharge
with the observation group lower than the control group
and the differences were statistically significant(P
<
0.05).At discharge
there were no significant differences in FEV
1
FVC levels
and CAT scores between the 2 groups(P
>
0.05).At 3 months after discharge
FEV
1
and FVC levels in the 2 groups were higher than those at discharge
with the observation group was higher than the control group
and the differences were statistically significant(P
<
0.05).At 3 months after discharge
the CAT score of patients in the 2 groups was lower than that at discharge
with the observation group lower than the control group
and the differences were statistically significant(P
<
0.05).Conclusion Out-of-hospital tracking management in combination with nutrition and health guidance for COPD patients with drug-resistant pulmonary tuberculosis can improve pulmonary function and quality of life
which hold promise for a wider application in clinical practice.
