Objective To analyze the correlation and prognostic value of albumin (Alb)
procalcitonin (PCT)
and amino-terminal brain natriuretic peptide precursor (NT-proBNP) in the detection of disease severity and prognosis assessment in patients with severe pneumonia-induced acute respiratory distress syndrome (ARDS).Methods 122 patients with severe pneumonia-induced ARDS admitted to our hospital from January 2019 to February 2022 were retrospectively analyzed and were set as the study group, and 118 patients who underwent physical examination in our hospital during the same period with normal conditions were selected as the control group. The levels of Alb,PCT,and NT-proBNP were compared in different populations and study groups with different degrees of illness and different prognoses,Spearman was used to analyze the correlation between Alb,PCT,NT-proBNP levels and APACHE II scores; multivariate Logistic regression analysis was used to analyze the multiple factors affecting the poor prognosis of severe pneumonia-induced ARDS patients and to calculate the sensitivity and specificity of different tests to predict the prognosis of severe pneumonia patients with ARDS. Results The level of Alb in the study group was lower than that in the control group, and the levels of PCT and NT-proBNP in the study group were significantly higher than those in the control group (P < 0.05). Alb level:severe<moderate < mild; PCT,NT-proBNP levels and APACHE II score:severe > moderate > mild, the difference was statistically significant (P < 0.05). Alb was negatively correlated with APACHE Ⅱ score, and PCT,NT-proBNP levels were positively correlated with APACHE Ⅱ score (P < 0.05). There were 96 cases with good prognosis and 26 cases with poor prognosis. There was no significant difference in gender, age,BMI,mean arterial pressure, and PH value between the two groups (P > 0.05). There were statistically significant differences in the levels of body temperature,WBC,neutrophils,Alb,PCT,and NT-proBNP between the two groups (P < 0.05). The results of multiple Logistic regression analysis showed that body temperature,WBC,neutrophils,Alb,PCT,and NT-proBNP levels were multiple factors affecting the poor prognosis of patients with severe pneumonia-induced ARDS (P < 0.05). The sensitivity and specificity of Alb,PCT and NT-proBNP in predicting the prognosis of severe pneumonia patients with ARDS were 92.12% and 88.64%,respectively,which were better than single detection. Conclusion By monitoring the changes of Alb,PCT and NT-proBNP levels in patients with severe pneumonia-induced ARDS,it was helpful to evaluate the progress of the disease which provide guidance for clinical treatment plans, and improve the prognosis.
