Objective To investigate the related factors affecting the prognosis of thrombectomy in stroke patients caused by proximal arterial occlusion in the anterior circulation，and to improve the prognosis of acute ischemic stroke（AIS）patients. Methods A retrospective analysis was performed on the clinical data of AIS patients with proximal vascular occlusion in the anterior circulation who underwent endovascular treatment at the stroke center of the First People’s Hospital of Zigong. The patients were divided into good prognosis group [modified Rankin Scale（mRS）≤ 2] and poor prognosis group（mRS > 2）according to mRS score. Logistic regression was used to analyze the related factors of prognosis. Results The univariate analysis showed that compared with the good prognosis group，the poor prognosis group had significantly higher proportion of patients with cardioembolic stroke or postinterventional cerebral hyperdensities，preoperative National Institutes of Health Stroke Scale（NIHSS）score，and the times of thrombectomy，as well as a sig-nificantly longer time from puncture to recanalization（all P < 0.05）. The multivariate analysis showed that high preoperative NIHSS score and increased times of thrombectomy were risk factors for poor prognosis（odds ratios = 1.238 and 2.528，95% confidence inter-vals: 1.075 ~ 1.425 and 1.342~4.726，respectively）. Conclusion The poor prognosis of endovascular treatment in stroke patients caused by proximal arterial occlusion in the anterior circulation is associated with relatively high preoperative NIHSS score and in-creased thrombectomy times.
