Objective To explore the clinical value of three-dimensional speckle tracking imaging in evaluating leftventricular systolic function in patients with aortic stenosis with different wall thickness.MethodsForty-five patients withsevere aortic stenosis with normal left ventricular ejection fraction(LVEF)were divided into stenosis group(20 cases
group A)and stenosis wall thickness group(25 cases
group B)according to the wall thickness.Another 30 healthy volunteers matched inage and sex were selected as control group. Conventional and three-dimensional echocardiography were performed to measureLVEF
left ventricular myocardial mass(LVM)
global peak longitudinal strain(GLS)
global circumferential strain(GCS)
globalarea strain(GAS)
global radial strain(GRS)
left ventricular mass index(LVMI).The differences of the above parameters of eachgroup were compared. Results GLS
GRS were significantly reduced
and GCS
LVMI were significantly increased in group A andgroup B compared with the normal control group. The differences were statistically significant(all P<0.01). There were nosignificant difference in the comparison between the LVEF and GAS in each group. There was significant difference in the GLS comparison between A group and B group(P<0.01)
but other parameters were no significant difference.GLS
GRS
GCS and GAS were positive correlated with LVMI(r=0.58
0.31
0.29
0.43
all P<0.05).Conclusion Three-dimensional speckle trackingimaging can assess impaired left ventricular systolic function in patients with normal LVEF aortic stenosis
and the cardiacfunction impaired is more obvious in patients with increased ventricular wall thickness and heavier myocardial mass.
