摘要

目的:探讨参麦注射液对老年晚期乳腺癌患者心力衰竭期心功能及血清N末端B型利钠肽原（NT-proBNP）、超敏心肌肌钙蛋白T（hs-cTnT）的影响。方法:选取2018年6月至2019年12月上海市宝山区中西医结合医院收治的心力衰竭期老年乳腺癌患者94例作为研究对象

采用随机数字法分为对照组（n=47）和观察组（n=47）。对照组患者采用常规对症治疗

观察组患者在对照组的基础上给予参麦注射液治疗。观察两组患者的临床治疗效果

治疗前后心功能指标如左心室射血分数（LVEF）、心排出量（CO）和舒张早期最大血流速度/舒张晚期最大血流速度比值（E/A）等

血清肌酸激酶同工酶（CK-MB）、肌酸激酶（CK）、NTpro BNP和hs-cTnT水平的变化。结果:治疗后

观察组患者的总有效率为87.23%（41/47）

明显高于对照组的65.96%（31/47）

差异有统计学意义（P<0.05）。治疗后

两组患者LVEF、CO及E/A水平均明显升高

且观察组患者明显优于对照组

差异均有统计学意义（P<0.05）。治疗后

两组患者CK-MB、CK、NT-proBNP和hs-cTnT水平明显降低

其中观察组患者的改善程度明显优于对照组

差异均有统计学意义（P<0.05）。结论:参麦注射液通过增强心肌收缩力及抗脂质过氧化作用显著改善老年晚期乳腺癌患者的心功能

降低心肌损伤及心力衰竭风险

提高治疗效果。

Abstract

OBJECTIVE: To explore the effects of Shenmai injection on heart function

serum N-terminal B-type natriuretic peptide（ NT-proBNP） and hypersensitive cardiac troponin T（ hs-cTnT） in elderly patients with advanced breast cancer at heart failure stage.METHODS: Totally 94 patients with advanced breast cancer at heart failure stage admitted into Shanghai Baoshan Integrated Hospital of Traditional Chinese and Western Medicine from Jun.2018 to Dec.2019 were extracted to be divided into the control group（ n = 47） and the observation group（ n = 47） via the random number table.The control group received conventional symptomatic treatment

while the observation group was treated with Shenmai injection on the basis of the control group.Clinical efficacy

changed of cardiac function such as left ventricular ejection fraction（ LVEF）

cardiac output（ CO）

early diastolic maximum flow velocity/late diastolic maximum flow velocity（ E/A）

serum creatine kinase isoenzyme（ CK-MB）

creatine kinase（ CK）

NT-proBNP and hs-cTnT levels before and after treatment were observed in two groups.RESULTS: After treatment

the total effective rate of the observation group was 87.23%（ 41/47）

significantly higher than 65.96%（ 31/47） of the control group

the difference was statistically significant（ P<0.05）.After treatment

the levels of LVEF

ratio of CO and E/A in two groups increased significantly

and the observation group was significantly better than the control group

with statistically significant difference（ P<0.05）.After treatment

the levels of CK-MB

NT-proBNP and hs-cTnT in two groups decreased significantly

and the improvement in the observation group was better than that in the control group

with statistically significant difference（ P < 0.05）.CONCLUSIONS: Shenmai injection can significantly improve the cardiac function of elderly patients with advanced breast cancer by increasing the myocardial contractility and the anti-lipid peroxidation

reducing the risk of myocardial injury and heart failure and improving the clinical efficiency.
