OBJECTIVE: To investigate the application and rationality of key monitoring drugs in Xianyang Central Hospital before and after intervention
so as to provide reference for standardizing the supervision of key monitoring drugs and promoting the rational drug use. METHODS: Drug information system was used to obtain the consumption sum
consumption amount
defined daily dose system (DDDs) and defined daily cost (DDC) of key monitoring drugs in our hospital before intervention (2018) and after intervention (2019). Meanwhile
through clinical pharmaceutical management software
1 161 and 1 131 medical records of patients discharged from our hospital with key monitoring drugs before and after intervention were selected respectively to evaluate and analyze the rationality of drug use from the aspects of indications
drug usage and dosage
contraindications and repeated drug use. RESULTS: Compared with before intervention
the consumption sum of key monitoring drugs in our hospital and the proportion to the total consumption sum of drugs showed the downward trend after intervention. The proportion of consumption sum of key monitoring drugs to the total consumption sum of drugs decreased from 6. 20% (15 483 950. 33 yuan/249 913 574. 40 yuan) to 3. 86% (10 455 436. 46 yuan/270 766 753. 90 yuan). After intervention
with the exception of gangliosides
osteopeptides and cinepazide maleate
the consumption sum of the other 10 key monitoring drugs decreased significantly. Before and after intervention
the DDDs of inverted sugar electrolyte and Danshen Ligustrazine ranked the top 2. After intervention
the ranking of DDDs of osteopeptide increased significantly. After intervention
the DDC of other key monitoring drugs decreased significantly except for the unchanged DDC of Danshen ligustrazine and cinepezil maleate. After intervention
the proportion of medical records with irrational application of key monitoring drugs decreased from 46. 77% (543/1 161) before intervention to 41. 03% (464/1 131)
yet there was still no clear indication of drug application. CONCLUSIONS: Our hospital has achieved certain results in the management of key monitoring drugs
yet strong measures are still needed to standardize the rational application of key monitoring drugs.
