OBJECTIVE: To compare the clinical effects of early application of levosimendan combined with recombinant human brain natriuretic peptide( rh-BNP)
in patients with acute coronary syndrome( ACS) after emergency percutaneous coronary intervention( PCI) for pump failure
so as to propose safe and effective intervention strategies.METHODS: Totally 86 patients with ACS of secondary pump failure after emergency PCI in the Third Hospital of Shandong from Feb.2018 to Feb.2019 were enrolled and randomly divided into the control group and the observation group via the random number table
with 43 cases in each group. The control group was treated with intraaortic balloon pump
while the observation group received levosimendan combined with rh-BNP as positive inotropic drugs
the course of treatment was 7 d.The observation indicators were respectively success rate and incidence of complication
left ventricular ejection fraction( LVEF)
left ventricular end-diastolic diameter( LVEDd)
serum BNP
myocardial enzymes and troponin levels
dopamine dosage
average length of stay
and incidence of major adverse cardiac events
overall survival rate and readmission rate after 6 months follow-up.RESULTS: The treatment success rate of the observation group and the control group was 95.3%( 41/43) and 93.0%( 40/43)
respectively
with no statistically significant difference( P > 0.05).The incidence of complications in the observation group was11.6%( 5/43)
significantly lower than that in the control group 30.2%( 13/43)
the difference was statistically significant( P<0.05).After treatment
the levels of LVEF in two groups were higher than those before treatment
and the serum BNP
myocardial enzymes and troponin levels decreased significantly. In the observation group
the levels of LVEF were significantly higher than those in the control group
and the serum BNP
myocardial enzyme and troponin levels were significantly lower than those in the control group
with statistically significant differences( P<0.05).The amount of dopamine in the observation group was significantly less than that in the control group
and the length of stay was significantly shorter than that in the control group
the differences were statistically significant( P < 0.05).The incidence of major adverse cardiac events and readmission rate in the observation group during follow-up were significantly lower than those in the control group
and the differences were statistically significant( P<0.05)
while the overall survival rate of two groups were not statistically significant( P>0.05).CONCLUSIONS: Intra-aortic balloon pump or positive inotropic drugs should be used as early as possible for pump failure in patients with ACS after emergency PCI
the overall effect is comparable.Positive inotropic drugs of levosimendan combined with rh-BNP can significantly reduce the incidence of recent complications and major adverse cardiac events
improve levels of BNP
myocardial enzymes and troponin
reduce the dosage of dopamine
promote the rapid recovery and reduce readmission.
