1944张肠外营养处方用药合理性分析
摘要
目的:探讨中山大学附属第七医院(以下简称"我院")肠外营养处方用药合理性
为下一步优化静脉用药调配中心(pharmacy intravenous admixture service
OBJECTIVE: To probe into the application rationality of parenteral nutrition prescriptions in the Seventh Affiliated Hospital of Sun Yat-sen University( hereinafter referred to as "our hospital")
so as to provide reference for further optimizing the review rules of pharmacy intravenous admixture service( PIVAS) and implementing pharmaceutical interventions. METHODS: Through HIS system
a total of 1 944 parenteral nutrition prescriptions from May 2018 to Sept.2019 were extracted. The stability and effectiveness of prescriptions were evaluated
including glucose concentration
amino acid concentration
cation concentration
addition of other therapeutic drugs
ratio of glucose to lipid
ratio of heat to nitrogen
and application of pharmacological nutrients. RESULTS: Of the 1 944 prescriptions
1 063 prescriptions were rational
with the rational rate of 54. 68%. There were 881 irrational prescriptions
including 457 prescriptions( 51. 87%) with irrational ratio of glucose to lipid
449 prescriptions( 50. 96%) with irrational ratio of heat to nitrogen
313 prescriptions( 35. 53%) with irrational amino acid concentration
153 prescriptions( 17. 37%) with irrational pharmacological nutrients
28 prescriptions( 3. 18%) with irrational addition of other therapeutic drugs
19 prescriptions( 2. 16%) with irrational glucose concentration
and 18 prescriptions( 2. 04%) with irrational cation concentration.CONCLUSIONS: There are some irrational phenomenon of parenteral nutrition prescriptions in the early stage of our hospital. Clinical nutritional pharmacists and PIVAS pharmacists should give full play to their professional skills and conduct real-time review and intervention on parenteral nutrition prescriptions to standardize the clinical application of parenteral nutrition and ensure the safety and effectiveness of patients’ medication.
