OBJECTIVE: To systematically evaluate the clinical efficacy and safety of Qili Qiangxin capsules in the treatment of acute myocardial infarction( AMI) complicated with heart failure( HF). METHODS: Chinese and English randomized controlled trials of Qili Qiangxin capsules in the treatment of HF complicated with AMI( the control group was treated with conventional western medicine
while the study group received Qili Qiangxin capsules in addition to conventional western medicine) published before Aug. 2019 were retrieved. The literature was screened according to inclusion and exclusion criteria
and the Cochrane Bias Risk Tool was used to assess methodological quality for Meta analysis. RESULTS: Totally 10 clinical studies were collected
which were all Chinese literature
1 048 patients were involved
including 530 cases in the study group and 518 cases in the control group. Meta analysis showed that the clinical efficacy of the study group was significantly better than that of the control group( RR = 1. 42
95% CI = 1. 21-1. 67
Z = 4. 23
P<0. 000 1)
the improvement of N-terminal B brain natriuretic precursor level was significantly better than that of the control group( SMD =-2. 02
95% CI =-3. 13--0. 91
P = 0. 000 4)
the improvement of left ventricular ejection fraction( MD = 7. 04
95% CI = 5. 10-8. 98
P < 0. 000 01)
left ventricular end-diastolic internal diameter( MD =-0. 94
95% CI =-1. 46--0. 43
P < 0. 000 01) and left ventricular end-systolic internal diameter( MD=-4. 18
95%CI=-5. 52--2. 84
P<0. 000 01) were better than those of the control group
6 minutes walking test distance was significantly longer than that of the control group( MD = 42. 68
95% CI = 23. 92-61. 44
P<0. 000 01)
the above differences were statistically significant. No severe adverse drug reactions were seen in either group. CONCLUSIONS: Qili Qiangxin capsules are safe and effective for improving the clinical symptoms and cardiac function of AMI complicated with HF
but its long-term prognosis still needs more clinical research.
