Objective:to investigate the therapeutic effect of single-stage posterior total spondylectomy and reconstruction in surgical treatment of thoracolumbar tumor. Methods:Nineteen patients with single thoracolumbar tumor were treated with single-stage posterior total spondylectomy and reconstruction including 13 males and 6 females aged from 18 to 71 years old(with an average of 56.5 years old)
There were 3 with giant cell tumor
1 with plasmacytoma 2 with notochordoma 5 with metastatic tumor of breast cancer 1 with lymphadenoma 1 with metastatic tumor of lung cancer
1 metastatic tumor of renal carcinoma
4 with metastatic tumor of prostatic carcinoma
and 1 with metastasis of gastroenteric tumor.the vertebrae with tumor were located from thoracic 3 to lumbar 3 according to Frankel neurological function classification
preoperative neurological function was grade C in 1 D in 5 and E in 13 all the patients underwent vertebrectomy through posterior midline approach anterior column reconstruction with titanium cage support in bone cement or cancellous bone grana posterior fixation through pedicle of vertebra and posterolateral autograft. Results:The mean operative time was 4.6 hours and the mean blood loss was 2 750 ml All the patients pulled through the period of perioperation safely and started out-of-bed activity from 5 to 7 days after operation Pain vanished completely in all cases after operation 17 cases were followed up from 19 months to 51 months with an average of 33.5 months In the latest followed-up the postoperative neurological function was grade E in all the 17 cases No tumor recidivation occurred in operative region.Conclusion:Single-stage posterior total spondylectomy and reconstruction in treatment of thoracolumbar tumor has the merits of thorough tumor removal
good spinal column stability reconstitution and low tumor recidivation in operative region which is a safe and effective technique for the treatment of spinal tumor.
