Objective: To investigate the prognosis value of lactate clearance rate in patients with respiratory failure.Methods:12 h lactate clearance rate was detected in 83 patients with respiratory failure.We used Logistic regression analysis for the prognosis.Results:A total of 17 patients died(20.5%).The sex
age
type of disease was not statistically significant between survival group and death group(P>0.05);the admission APACHE Ⅱ score
pH value
PaO2
PaCO2
PaO2/FiO2
admission blood lactate and lactic acid 12h clearance rate were significant differences between two groups(P<0.05).The blood lactate was negative correlated with pH value
PaO2
PaO2/FiO2
and positively correlated with PaCO2
APACHE Ⅱ
while the 12h lactate clearance rates were more apparent with these factors.The mortality in high lactate clearance rate group was 13.0%
Vs 34.5% in low clearance rate group(χ2=5.365
P= 0.021).The low-clearance group had a higher risk of mortality
OR= 3.534
95% CI was 1.023~12.524.After Logistic regression multivariate adjustment
the OR was 2.782.Conclusion: The rate of lactate clearance in patients with respiratory failure could be used as an important indicator for prognosis.
