Objective:To explore the related complications and management in the treatment of intertrochanteric fractures of femur with proximal femoral nail(PFN).Methods:42 cases of intertrochanteric fractures of femur treated with PFN that could be followed up during September 2005 to May 2010 in my hospital were retrospectively analyzed. There were 18 males and 24 females with an average age of 78 years(ranging from 51 to 90 years).According to Tronzo-Evans classification
there were 3 cases for typeⅠ
15 cases for typeⅡ
10 cases for typeⅢ
9 cases for typeⅣ
and 5 cases for typeⅤ.Results:All the patients were followed up for a mean time of 30 months(ranging from 5 to 60 months). No wound infection
DVT
coxa vara or ANFH occurred in these cases. One patient presented with proximal femoral fracture in the operation. Two patients suffered cutting out and screw breakage. One patient complained of pain in the injured buttock and great trochanter. Conclusion:Exact reduction
strict aseptic operation
reasonable application of antibiotics
ideal insertion of internal fixators
active treatments for osteoporosis
individual healing exercise and comprehensive prevention of DVT are the keys to avoiding the complications.
