摘要

目的:观察纤维支气管镜肺泡灌洗术在重症肺部感染中的治疗效果。方法:总结我科近4年来重症肺部感染患者行纤维支气管镜（简称纤支镜）支气管肺泡灌洗（Bronchoalveolar lavage

BAL）治疗的体会

并和常规内科治疗进行临床疗效、住院天数和痰细菌培养阳性率的对比。结果:BAL治疗组总有效率97.9%

对照组（常规内科治疗组）总有效率83%

BAL组疗效明显高于对照组（P<0.05）;BAL组和对照组痰细菌培养阳性率分别是87.5%和34%

两组痰细菌培养阳性率比较有显著性差异（P<0.05）;BAL组和对照组平均住院天数分别是14.5 d和23.8 d

BAL组住院天数比对照组明显缩短。结论:重症肺部感染患者使用纤支镜肺泡灌洗能提高治疗效果

缩短住院时间

减少患者痛苦

值得推广。

Abstract

Objective:To observe the therapeutic efficacy of bronchoalveolar lavage(BAL) in the treatment of severe pulmonary infection.Methods:95 cases of severe pulmonary infection patients in our department in recent 4 years were collected

control group consist of 47 cases under routine medical treatment only

while BAL treated group consist of 48 cases both accepted routine medical treatment and BAL.To compare the clinical efficacy

hospitalization days

and positive rate of sputum bacterial culture of the two groups.Results:The total effective rate in BAL group(97.9%) was higher than that of control group(83%) obviously;the positive rate of sputum bacterial culture in BAL group(87.5%) was also higher than control group(34%)

both have significant difference

P<0.05;The average days of hospitalization in BAL group were 14.5 d

shorter than control group(23.8d). Conclusion:BAL can enhance the therapeutic efficacy in the treatment of severe pulmonary infection

shorten the length of stay

relieve the suffering

and is worth promotion.
