摘要

目的:探讨影响大肠癌合并糖尿病患者术后并发症发生的危险因素。方法:对2006年1月～2009年12月我院普外科收治的59例大肠癌合并糖尿病患者进行回顾性分析。采用χ2检验对10个临床相关因素进行单因素分析

再进一步行多因素Logistic回归分析。结果:59例患者中24例出现并发症

其发生率为40.7%。单因素分析结果提示合并症、糖化血红蛋白（Glycosylated hemoglobin

HbAc）、体质指数（Body mass index

BMI）、胰岛素抵抗指数（Insulin resistance index

IRI）与术后12h血糖为危险因素（P<0.05）。进一步的多因素Logistic回归分析结果表明术后血糖与稳态模型胰岛素抵抗指标检测（Homeostasis model assessment insulin resistance index

HOMA-IR）是独立的危险因素（P<0.05）。结论:影响大肠癌合并糖尿病患者术后并发症发生率的主要危险因素为术后血糖与胰岛素抵抗。

Abstract

Objective:Correlated factor analysis of postoperative complication of colorectal carcinoma combined with diabetes.Methods:From Jan 2006 to Dec 2009

59 cases of colorectal carcinoma combines with diabetes in our hospital were analyzed retrospectively.The risk factors were examined by chi-square test and multivariate logistic regression analysis.Results:The overall operative complication rate was 40.7%.Univariate analys showed that major risk factors of postoperative complication of the patients were other system diseases

glycosylated hemoglobin

body mass index

insulin resistance index and postoperative serum glucose(P<0.05).Multivariate logistic regression analysis showed that insulin resistance index and postoperative serum glucose were independent risk factors(P<0.05).Conclusion:Postoperative complication of colorectal carcinoma combined with diabetes are closely related to insulin resistance index and postoperative serum glucose.
