摘要

目的:研究在意义不明的不典型鳞状细胞（Atypical squamous cells of undetermined significance

ASC-US）

不能排除高度上皮内瘤变的不典型鳞状细胞（Atypical squamous cells

cannot exclude high-grade squamous intraepithelial lesion

ASC-H）

低度鳞状上皮内病变（Low-grade squamous intraepithelial lesion

LSIL）

高度鳞状上皮内病变（High-grade squamous intraepithelial lesion

HSIL）中p16INK4a蛋白和HPV16/18的表达水平及意义。方法:应用免疫组织化学技术和原位分子杂交技术

对89例子宫颈细胞涂片样本中p16INK4a蛋白及HPV16/18型DNA进行检测。结果:p16INK4a蛋白阳性率随着子宫颈病变程度的加重而升高

LSIL、HSIL及ASC-H的p16INK4a蛋白阳性率均高于ASC-US（P<0.05）;ASC-US和LSIL的p16INK4a阳性表达与HPV16/18阳性表达无差异性;LSIL和ASC-US的HPV16/18阳性表达无差异性。结论:子宫颈细胞涂片样本中p16INK4a蛋白检测能协助子宫颈细胞学诊断分级

尤其对于ASC-US的病例

需同时做p16INK4a免疫组织化学检查

以帮助临床医生选择恰当的处理方式。

Abstract

Objective:To investigate the expression and significance ofp16INK4a and HPV16/18 in atypical squamous cells of undetermined significance(ASC-US)

atypical squamous cells

cannot exclude high-grade squamous intraepithelial lesion(ASC-H)

low-grade squamous intraepithelial lesion(LSIL)

high-grade squamous intraepithelial lesion(HSIL).Methods:p16INK4a and HPV16/18 DNA in thin-layer slides from 89 specimens were detected by immunohistochemistry and the insitu hybridization.Results:A positive correlation was found between the positive rate of p16INK4a and cervical disease progression

the positive rate of p16INK4a in LSIL

HSIL

ASC-H were higher than ASC-US(P<0.05);There were not variability between the positive rate of p16INK4a and HPV16/18 DNA in ASC-US and LSIL; p16INK4a positive rate in ASC-US was not related with p16INK4a positive rate in LSIL.Conclusion:Detecting p16INK4a in thin-layer slides could help staging in cervical cytology

p16INK4a should be detected by immunohistochemistry for ASC-US

which could help clinical doctor choose a curative treatment.
