摘要

目的 :探讨抑肽酶对食管癌切除围手术期的凝血—纤溶系统的影响。方法 :6 0例食管癌切除手术患者随机分为抑肽酶治疗组（A）和对照组（B）

检测两组不同时间血液凝固启动时间（CST）、最大凝固程度（MCE）、纤维蛋白完全溶解时间（WFT）的改变。结果 :治疗组CST在术后 2h、2 4h明显减少

与对照组比较有极显著差异（P <0 .0 1）。而术前及术后 4 8h以上两组没有显著差别 ;MCE、WFT在术前 30min及术后 2h、2 4h均明显高于对照组（P <0 .0 1）

而术后 4 8h后与对照组比较无显著差异 ;术中失血量及术后 2 4h内胸腔引流量亦明显少于对照组（P <0 .0 1）。结论 :食管癌围手术期使用抑肽酶能有效的调节凝血—纤溶系统

对防止术中及术后出血有显著的疗效。

Abstract

Objective:To study the effect of aprotinin on blood coagulation and fibrinolysis system.Methods:60 cases of esophagectomy were randomly divided into aprotinin group(A) and control group(B)

and the results of Coagulate Start Time(CST)

Most Coagulate Extent (MCE)

Whole Fibrinolytic Time (WFT) were detected in different time(30 minute before operation and 2h

24h

48h

72h after operation).Results:Compared with group B

CST in 2h

24h after operation was reduced significantly in group A( P <0.01)

but there were no significant differences before operation and 48h

72h after operation.Meanwhile

MCE and WFT were markedly increased in 30 minute before operation and 2h

24h after operation( P <0.01)

while there were no significant differences 48h after operation.Meanwhile

compared with group B

the blood lost in group A during operation and after operation was much less( P <0.01).Conclusion:Aprotinin may effectively regulate blood coagulation and fibrinolysis system during esophagectomy

and markedly ameliorate the blood loss.
