摘要

目的 :探讨重症肌无力胸腺切除术后机械通气的危险因素。方法 :1987年 1月至 2 0 0 3年 8月

我科共收治重症肌无力患才 4 2例

运用统计软件对年龄、性别、病程、临床分型、病理类型、手术时间、术前吡啶斯的明用量、术前激素的使用等因素进行多因素logistic回归模型分析。结果 :影响机械通气的主要危险因素有 :临床分型、病理类型

且呈正相关。结论 :重症肌无力胸腺切除术后临床分型、病理类型等因素可作为是否延长机械通气的选择指征

Abstract

Objective:To explore the risk factors in mechanical ventilation on patients with myasthenia gravis after thymectomy.Methods:42 patients with myasthenia gravis were treated by thymectomy from January 1987 to August 2003.Using logistic regression model

a multi-variate analysis was made with regard to age

sex

disease history

Osserman classication

pathological types

the consumed time of operation

the preoperatie dose of pyridostigmine

the preoperative usage of hormone.Results:The major risk factors that affected mechanical ventilation on patients with myasthenia gravis after thymectomy were Osserman classication and pathological types.Conclusion:Osserman classication and pathologic types might serve as indices for the prolonged mechanical ventilation.
