摘要

目的 :探讨影响正常糖耐量（NGT）人群胰岛素抵抗（IR）和胰岛 β细胞分泌功能的相关因素。方法 :对 2 12 0例NGT者

用稳态模式评估法（HOMA）评价胰岛素抵抗和胰岛 β细胞分泌功能。结果 :（1）单因素方差分析表明

不同年龄组的HOMA -IR、HOMA - βcell有显著性差异（P <0 .0 5 ）。（2 ）t检验表明

超重 /肥胖组较正常体重组

中心性肥胖组较非中心性肥胖组、高TG组较正常TG组、高血压组较正常血压组HOMA -IR、HOMA - βcell均有显著性差异（P <0 .0 1）。（3）多元逐步回归分析表明

年龄、BMI、腰围、TG和舒张压是影响HOMA -IR、HOMA - βcell的主要因素。结论 :NGT人群随着年龄、BMI、腰围、TG和血压的增加

IR加重 ;胰岛 β细胞分泌功能随着年龄的增加而减弱

随着BMI、腰围、TG和血压的增加而代偿性增强

以维持正常的糖耐量。

Abstract

Objective:To investigate the factors related to insulin resistance and islet β cell secretory function in subjects with normal glucose tolerance(NGT).Methods:Insulin resistance and islet β cell secretory function were estimated by Homeostasis Model Assessment in 2120 NGT subjects.Results:(1)ANOVA showed that there were significant differences in Homa-IR and Homa-β cell among different age groups(P<0.05).(2)t test indicated that there were significant differences in HOMA-IR and HOMA-β cell in overweight/obesity group

high TG group and hypertension group compared with their control groups respectively(P<0.01).(3)Stepwise regression revealed that the main factors influencing HOMA-IR and HOMA-β cell included age

TG and diastolic blood pressure.Conclusion:In NGT population

IR is aggravating with the age

waist circumference

TG and blood pressure;islet β cell secretory function is weakening with aging

while it is enhancing with BMI

waist circumference

TG and blood pressure to maintain normal glucose tolerance.
