Objective:To discuss the clinical characteristics of neuromyelitis optica spectrum disorders(NMOSD)with a first attack of intractable hiccup and nausea(IHN). Methods:Clinical data on two patients with NOMSD with a first attack of IHN were summarized and the related literature was reviewed. Results:Both of the two cases presented IHN during the first attack of the disease
and clinical relapses occurred within a short time. In case 1
the patient developed brainstem manifestations and myelitis two weeks later and developed optic neuritis six months later. Case 2 patient relapsed and presented with longitudinally extensive transverse myelitis(LETM)two months after the first attack. Serum aquaporin-4(AQP4)antibodies were positive in both the two patients
supporting the diagno sis of NMOSD. Conclusion:NOMSD with a first attack of IHN is often diagnosed late or misdiagnosed.
