Objective:To evaluate the clinical efficacy of closed reduction and percutaneous cannulated screw fixation in the treatment of Salter-Harris type Ⅲ epiphyseal fracture of the medial malleolus of children. Methods:Twenty-eight cases of type Ⅲ epiphyseal fracture of the medial malleolus of children from January 2011 to August 2015 were enrolled
including 20 males and 8 females aged from 9 to 13 years old
with average of 11 years and 3 months. According to Salter-Harris classification criteria of epiphyseal injuries
the medial malleolus epiphyseal fractures were type Ⅲ. The 28 cases were treated by closed reduction under the help of X-ray. Then the medial malleolus fractures were fixed with percutaneous minimally invasive cannulated screws. All cases had been treated with postoperative external fixation of ankle by plaster or brace for 3-4 weeks
and then gradually weight-bearing ambulated
and walked after removing external fixation. Results:All cases were followed up for average of 25 months. All cases achieved bone union after 3months. The symptoms of ankle joint deformity
pain and instability in the postoperative follow up for about 2 years were not found. At the same time
there was no early closure or bone bridge formation of epiphyseal
and no uneven articular surface. All the children can participate in normal sports activities. The result of Baird-Jackson ankle score in all cases obtained excellence in 24 cases
good in 4cases
and the fine rate achieved 100%. Conclusion:The method of closed reduction and percutaneous cannulated screw fixation treating Salter-Harris type Ⅲ epiphyseal fracture of the medial malleolus of children can obtain minimal trauma
reliable fixation and satisfactory efficacy. So it is an effective and alternative clinical treatment method.
