Objective:To analyze and evaluate the implementation of clinical pathway for neonatal respiratory distress syndrome(NRDS). Methods:Children who diagnosed as NRDS in the Children Hospital of Chongqing Medical University were enrolled.Children enrolled from January 1
2013 to December 30
2014 and received clinical pathway management were taken as pathway management group;children enrolled from January 1
2011 to December 30
2012 but did not receive clinical pathway management were taken as non-pathway management group. The hospitalization expenses
length of hospital stay
respirator using time
incidence of ventilator associated pneumonia(VAP)
hospital infection and the satisfaction of children ’s parents were evaluated between two groups
and the variation of clinical pathway was analyzed. Results:There were 1 074 eligible cases in total
including 494 cases for clinical pathway management group and 580 cases for non-pathway management. There was no significant difference in age
gender
gestational age
birth weight
chest radiograph classification between two groups(P>0.05). Hospital cost
length of hospital stay and respirator using time were decreased after pathway management
besides occurrence of ventilator associated pneumonia was reduced. But there were no significant difference in hospital infection and the satisfaction of children’s parents between two groups(P>0.05).A total of 159 patients completed the clinical pathway
and variation was found in 315 cases. The main reason for variation was complications of NRDS(patent ductus arteriosus is the most common complications). Conclusion:For neonates with NRDS
clinical pathway management can reduce the length and expenses of hospitalization
as well as guarantee the efficacy and satisfaction of children’s parents. But it still needs improvement due to complications of NRDS and high variation rate.
