Objective To compare the efficacy of cognitive behavioral therapy(CBT)
drug treatment versus their combination for post-stroke sleep disorder. Methods A total of 160 patients with post-stroke sleep disorder admitted into the Department of Neurology of a Henan hospital between December 2016 and March 2017 were included in this study.Based on a random number table
patients were divided into three groups to receive CBT
estazolam
or combination therapy(CBT+estazolam)in addition to routine treatment. The treatment duration was 4 weeks. The sleep quality was assessed by Pittsburgh Sleep Quality Index(PSQI)before treatment
at 2 and 4 weeks of treatment
as well as at 2 and 4 weeks of follow-up.Results The pre-treatment PSQI score was similar across the three groups(P>0.05).At 2 weeks of treatment
the PSQI score was decreased in all groups compared with their pre-treatment scores(P<0.05);the PSQI score in the combination group was significantly lower than that in the other two groups(P<0.05)
and the score in the estazolam group was significantly lower than that in the CBT group(P<0.05).At 4 weeks of treatment
the PSQI score was further decreased in all groups compared with their pre-treatment scores(P<0.05)
but without any significant difference among the three groups(P>0.05).After treatment completion
the PSQI score in the estazolam group tended to increase
while that in the CBT group and the combination group generally remained unchanged. At 2 weeks of follow-up
the PSQI score in the estazolam group was significantly higher than its score at 4 weeks of treatment as well as that in the combination group(P<0.05).At 4 weeks of follow-up
the PSQI score in the estazolam group was also significantly higher than its score at 4 weeks of treatment as well as that in the combination group and the CBT group(P <0.05).Conclusion For patients with poststroke sleep disorder
drug treatment alone has obvious immediate effect and its short-term efficacy is favorable. The long-term effect of CBT alone is superior to drug alone
but its immediate effect is inferior to drug alone. The combination of both seems to an ideal option for the treatment of post-stroke sleep disorder.
