Application of small incision closure through right chest and right atrium in children with ventricular septal defect
Objective To explore the application of small incision closure through right chest and right atrium in children with ventricular septal defect. Methods The clinical data of 174 children with ventricular septal defect from May 2014 to July 2018 in Anhui children′s hospital were retrospectively reviewed.Of whom 114 cases underwent small incision closure through right chest and right atrium were regarded as occlusion group. The remaining 60 cases underwent open heart surgery under conventional cardiopulmonary bypass were regarded as repair group. The related surgical indicators
changes of hemoglobin
platelet
leukocyte and platelet aggregation function before and after operation
changes of pulmonary function and left ventricular function before and 3 months after operation
and incidences of complications within 3 months after operation were compared between the two groups. Results There was a significant difference in the defect types between the two groups(P<0.05).The operation time
CICU monitoring time and total hospitalization time of the occlusion group were shorter than those of the repair group, the differences were significant(P<0.05).The blood transfusion volume
red blood cell transfusion volume of the occlusion group were less than those of the repair group, the differences were significant(P<0.05).And the operation cost of the occlusion group was more than that of the repair group
the differences were significant(P<0.05).After operation
hemoglobin and platelet in both groups decreased
the levels of hemoglobin and platelet in the occlusion group were higher than those in the repair group
the differences were significant(P<0.05).And the level of white blood cells in occlusion group was higher than that in repair group
while that in occlusion group was lower than that in repair group
the difference was significant(P<0.05).There was no significant change in platelet aggregation function in the occlusion group compared with that before suregry(P>0.05)
but the platelet aggregation function in the repair group increased
which was higher than the that in occlusion group
the differences were significant(P<0.05).The levels of FVC
LVEF
LVEDV and LVESV increased in 3 months after operation
while the level of FEV1 decreased
the differences were significant(P<0.05).And pulmonary function and left ventricular function were similar in the two groups within 3 months after surgery;the incidence of complications in occlusion group was lower than that in repair group within 3 months after operation
the differences were significant(P<0.05).Conclusion Small incision closure via right chest and right atrium in children with ventricular septal defect has the advantages of minimal trauma and quick recovery. It can promote the recovery of pulmonary function and left ventricular function after operation
which is similar to traditional operation
but the operation costs is higher.
