Application of enhanced recovery after surgery in bridging replantation of severed fingers with perforator bone flap of perforator of anterior tibial artery
Objective To explore the efficacy of enhanced recovery after surgery(ERAS) in bridging replantation of severed fingers with perforator bone flap of anterior tibial artery. Methods A total 68 patients with amputated fingers who received bridging replantation of perforator bone flap of anterior tibial artery in a hospital in Jiaozuo from January 2019 to June 2020 were randomly divided into study group and control group
with 34 cases in each group. Patients in control group were treated with routine nursing and patients in study group were treated with ERAS. The recovery of blood supply
the survival rate and the incidence of complications of amputated fingers at 7 days after operation
the effect of intervention 3 months after operation
the motor function and quality of life of severed fingers 1 year after operation were compared between the two groups. Results The capillary filling time and VAS score of amputated fingers in the study group were lower than those in the control group
and the oxygen saturation in the study group was significantly higher than that in the control group
and the difference was statistically significant(P<0.05). The survival rate of severed fingers in the study group(89.58%
43/48) was higher than that in the control group(69.23%
36/52)
and the difference was statistically significant(χ~2=6.232
P=0.013). The excellent and good rate of patients in the study group after treatment for 3 months was(88.24%
30/34)
which was higher than that of the control group(64.76%
31/34)
and the difference was statistically significant(χ~2=6.353
P=0.012). One year after operation
the finger grip strength index
range of motion
dexterity and Carroll score of the affected fingers in the study group were significantly higher than those in the control group
and the difference was statistically significant(P<0.05). One year after operation
the scores of physiological
psychological
environmental and social relations in the WHO-QOL100 scale of the study group were significantly higher than those of the control group
and the difference was statistically significant(P<0.05). Conclusion After bridging replantation of severed fingers with perforator bone flap of anterior tibial artery
ERAS intervention is helpful to improve the blood supply of amputated finger
relieve pain
improve the survival rate of amputated finger
improve the motor function of affected finger and improve the quality of life.
