摘要

医养融合是应对人口老龄化的国家战略

实施过程需要医疗与养老跨职能团队的密切协作融合。在老龄化程度不断加剧、健康医养协同推进的过程中

医疗与养老跨职能团队的协作与融合面临更多的挑战。医疗和养老团队在文化素养、专业知识和目标导向之间的较大差异是影响公立医院医疗与养老跨职能团队协作融合的主要因素。本文以试点公立医院为研究对象

以创建全国敬老文明号、全国老年友善医院、安宁疗护试点单位、老年营养示范病房为研究路径

构建了以文化为前提、以共情为基础、以老人为中心、以专业为支撑的医疗养老跨职能团队的医养融合模式。

Abstract

The integration of medical care and eldercare is a national strategy to cope with population aging

and the implementation process requires the close cross-functional collaboration between the medical and eldercare teams. This integration faces more challenges in the process of increasing aging and the synergistic promotion of health care and health maintenance. The large differences in cultural literacy

expertise

and goal orientation between the medical and eldercare teams are the main factors affecting the cross-functional collaboration of medical and eldercare teams in public hospitals. In this paper

we take a pilot public hospital as the research subject and by creating a national model hospital for respect of the elderly

a national elderly-friendly hospital

a hospice pilot unit

and a demonstration ward for elderly nutrition

establish a culture and empathy-based

elderly-centered

and professionally supported cross-functional collaboration model between the medical and eldercare teams.
