The effect of hospital-home coordinated seamless continuous care on psychological status self-management and quality of life in elderly patients with coronary heart disease undergoing PCI
Objective To investigate the effect of hospital-home coordinated seamless continuous care on the psy-chological status
self-management and quality of life in elderly patients undergoing percutaneous coronary inter-vention(PCI) for coronary heart disease. Methods One hundred and forty-eight elderly patients with coronary artery disease who underwent PCI from October 2018 to February 2020 in a tertiary hospital in Nanyang City were selected as the study subjects
and they were divided into a control group(74 cases) and an observation group(74 cases) using a random number table. Patients in the control group were given regular nursing care
and patients in the observation group were given hospital-home coordinated seamless continuous care in addition to the nursing care in the control group. The psychological status as reflected in Hamilton Anxiety Scale(HA-MA) and Hamilton Depression Scale(HAMD) scores
quality of life as reflected in Minnesota Living with Heart Failure Questionnaire(MLHFQ) scores
and self-management as reflected in the Coronary Artery Disease Self-Management Scale(CSMS) scores were compared between the two groups at admission
discharge
and 3 months after discharge. Results There were 41 intraoperative adverse events in the control group
and the inci-dence of adverse events was 55.41%. There were 14 cases of intraoperative adverse events in the observation group
with an incidence of 18.92%. The incidence of intraoperative adverse events in the observation group was lower than that in the control group
and the difference was statistically significant(P<0.05). At the time of admission
the HAMA
HAMD
MLHFQ
and CSMS scores of the two groups were compared
and the differ-ences were not statistically significant(P>0.05). At the time of hospital discharge and 3 months after hospital discharge
the HAMA
HAMD
and MLHFQ scores of patients in the observation group were significantly low-er than those in the control group
and the differences were statistically significant(P<0.05). The CSMS scores of patients in the observation group were significantly higher than those in the control group at the time of discharge from the hospital and at 3 months after discharge
and the difference was statistically significant(P<0.05). Conclusion Hospital-home coordinated seamless continuous care can effectively improve the negative e-motions of elderly patients with coronary heart disease undergoing PCI
reduce the incidence of intraoperative ad-verse events
and improve patients’ quality of life and self-management.
