Objective To assess the impact of the family physician team on the health management of patients with type 2 diabetes mellitus in the community through contract signing. Methods One hundred patients with type 2 diabetes mellitus who visited a hospital in Xi’an from January 1
2018 to July 1
2018 were selected as the study subjects
and the subjects were divided into a study group and a control group with 50 cases in each group using a random number table. The patients in the study group were managed by a signed family physician team
while the control group was managed in a routine outpatient clinic. The cognitive status regarding diabetes mellitus
self-management ability
body mass index and blood glucose control of the two groups were compared. Results After health management
the cognition of diabetes mellitus and self-management ability scores of the patients in the study group were(7.25±1.04) and(6.56±0.84) points
respectively
which were significantly higher compared with those before health management(P< 0.05)
while the body mass index and blood glucose index(fasting blood glucose
2 h postprandial blood glucose
glycated hemoglobin) were(23.88±2.74) kg/m2
(5.77±0.79) mmol/L
(9.26±1.36) mmol/L and(5.87±0.80) %
respectively
all significantly lower than those before health management(P< 0.05). Besides
after health management
the scores in the cognition of diabetes mellitus and self-management in the study group were higher than those in the control group
while the body mass index and blood glucose index were lower than those in the control group in the same period
and the differences were statistically significant(P< 0.05). Conclusion The community health management of patients with type 2 diabetes mellitus with signed family physician team as the main body can significantly improve the cognitive and self-management abilities
