摘要

目的探讨老年尿毒症血液透析患者的自杀意念状况及影响因素。方法选取2017年3月—2019年3月于河南省某医院就诊的114例老年尿毒症血液透析患者为研究对象

采用自杀意念自评量表(SIOSS)评估患者的自杀意念状况

同时收集患者一般资料并采用Logistic回归分析影响老年尿毒症血液透析患者出现自杀意念的危险因素。结果 114例患者中有18例出现自杀意念

发生率为15.79%;老年尿毒症血液透析患者"有自杀意念"发生率在年龄方面的差异无统计学意义(P>0.05);老年尿毒症血液透析患者"有自杀意念"发生率在性别、并发症、透析年限、文化程度方面的差异有统计学意义(P<0.05)。Logistic回归分析结果表明

高中及以上文化程度以及发生并发症是老年尿毒症血液透析患者出现自杀意念的危险因素(P<0.05)。结论老年尿毒症血液透析患者自杀意念的发生与性别、并发症发生情况以及透析年限和受教育程度相关

应采取针对性干预措施

注重人文关怀

加强心理疏导

提升患者生存质量。

Abstract

Objective To investigate the status of suicidal ideation and its influencing factors in elderly patients with uremia treated with hemodialysis.Methods One hundred and fourteen elderly patients with uremia treated with hemodialysis in a hospital in Henan Province from March 2017 to March 2019 were selected as the research subjects.The Seff-ratingidta of suicide scale（SIOSS）was used to evaluate the suicide ideation of the patients.At the same time

the general data of patients were collected and the logistic regression analysis was used to analyze the risk factors affecting suicidal ideation in elderly patients with uremia treated with hemodialysis.Results In the 114 patients

18 had suicidal ideation

the incidence was 15.79%.There was no significant difference in the incidence of"suicidal ideation"in elderly patients with uremia treated with hemodialysis in terms of age（P>0.05）.There was significant difference in the incidence of"suicidal ideation"in elderly patients with uremia treated with hemodialysis in terms of gender

complications

duration of dialysis

and education（P<0.05）.Logistic regression analysis showed that high school education and above and complications were independent risk factors for suicidal ideation in in elderly patients with uremia treated with hemodialysis（P<0.05）.Conclusion The occurrence of suicidal ideation in elderly patients with uremia treated with hemodialysis is related to gender

complications

dialysis duration and education level.Therefore

targeted intervention measures should be taken for patients with higher education level and complications

which can improve patients’ psychological state and quality of life.
