Efficacy and safety of concurrent chemoradiotherapy combined with Taixinsheng in the treatment of advanced esophageal cancer
Objective To investigate the clinical efficacy and safety of concurrent chemoradiotherapy combined with Taixinsheng(tradename for nimotuzumab)in the treatment of advanced esophageal cancer. Methods A total of 106 patients with advanced esophageal cancer admitted to a hospital in Liaoning from October 2014 to July2015 were selected as subjects.Patients were divided into control group(n=66)and observation group(n=40).Both groups were treated with chemotherapy combined with three-dimensional conformal radiotherapy
while observation group was treated with Taixinsheng in addition. The therapeutic effects of the two groups were compared at 3 days after treatment;the level of carcinoembryonic antigen(CEA)
high mobility protein B1(HMGB1)and squamous cell carcinoma antigen(SCC-Ag)were compared between the two groups before treatment and at 1 week after the treatment;at the time of discharge
the adverse reactions of the two groups were compared;the survival of the two groups was compared for 3 years. Results The objective response rate of the observation group was 92.50%
which was higher than that of the control group(77.27%).The differences were statistically significant(χ2=4.096
P=0.043).The disease control rate of the observation group and the control group were 100.00%and 95.45%
respectively
and the difference was not statistically significant(P
>
0.05).At the end of treatment
the levels of CEA
HMGB1 and SCC-Ag in the observation group were(46.78±5.01)ng/mL
(104.12±11.26)ng/mL
(26.35±2.74)ng/mL
respectively
which were lower than those of the control group
which were(58.24±5.93)ng/mL
(134.15±14.38)ng/mL and(42.96±4.37)ng/mL;the difference was statistically significant(P
<
0.05).The incidences of fever and rash in the observation group were57.5% and 65.0%
which were higher than those of the control group(30.3% and 21.2%).The difference was statistically significant(χ
2
=7.641
20.324;P
<
0.05).The 1-year survival rate of the observation group was72.50%
which was higher than that of the control group(57.58%).The difference was statistically significant(χ
2
=4.254
P
<
0.05).There was no significant difference in the 2-year survival rate and 3-year survival rate between the two groups(67.50% and 22.50%in the observation group
46.97%and 16.67%in the control group;P
>
0.05).The median survival time was 29 months(95%CI:26.2 to 31.8)in the observation group and 19 months(95%CI:7.4 to 30.6)in the control group. Conclusion Concurrent chemoradiotherapy combined with Taixinsheng has a definite curative effect on advanced esophageal cancer. It can improve the objective remission rate
tumor marker level and the 1-year survival rate of patients with advanced esophageal cancer
and has a favorable safety profile. Thus
it is worthy of clinical application.
