Effects of fecal microbiota transplant combined with lactulose on the gastrointestinal function and quality of life in patients with intractable functional constipation
Objective To investigate the effects of fecal microbiota transplant(FMT)combined with lactulose on the gastrointestinal function
mental state and quality of life in patients with intractable functional constipation. Methods 100 patients with intractable functional constipation diagnosed and treated from January 2017 to December 2018 in a hospital in Shenyang were selected as study subjects. The patients were divided evenly into observation group and control group by random number table. Both groups were given oral lactulose
while the observation group was treated with FMT in addition. After treatment
the recovery of gastrointestinal function
changes in the levels of motilin(MTL)
substance P(SP)
vasoactive intestinal peptide(VIP)and the scores of Knowles Eccersley Scott Symptom(KESS)scoring system
Patient Assessment of Constipation Quality of Life(PAC-QOL)questionnaire
Self-Rating Anxiety Scale(SAS)and Self-Rating Depression Scale(SDS)in the two groups were compared. Results The time of first anus exhaust
return of bowel sound and first independent defecation in the observation group were(11.42±2.65)h
(47.04±12.11)h and(60.38±3.92)h
respectively. They were significantly shorter than those in the control group
which were(23.58±2.49)h
(92.87±18.87)h and(95.51±18.75)h
respectively(P<0.05).After treatment
the levels of MTL
SP and VIP in the observation group were(239.42±40.39)pg/mL
(45.03±5.93)pg/mL and(12.18±2.57)pg/mL
respectively
and those in the control group were(195.83±39.46)pg/mL
(36.72±6.02)pg/mL
(15.74±2.08)pg/mL
respectively. The MTL and SP levels in the observation group were significantly higher than those in the control group
while the VIP level was significantly lower(P<0.05).The scores of KESS
PAC-QOL
SAS and SDS in the observation group were 12.67±6.85
62.35±20.48
40.61±5.46
and 39.75±6.11
respectively. All scores were significantly lower than those in the control group
which were 17.21±7.24
83.51±21.05
45.82±6.73
and46.41±5.38
respectively(P<0.05).Conclusion Fecal microbiota transplant combined with lactulose in the treatment of intractable functional constipation not only maintains sufficient water in the intestine
but also improves the coordination and contraction of the anorectal and rectal compliance
thus shortening the time of first anal exhaust
return of bowel sound and first independent defecation. It also relieves the bad mood of patients and improves their quality of life.
