Analysis of 5 years accumulative survival of patients with acute coronary syndrome
Objective To study the factors influencing 5years cumulative survival of patients with acute coronary syndrome(ACS).Methods 285 patients with ACS from a hospital in Pingdingshan city were selected as research objects
and on admission patients′gender
age
body mass index(BMI)
hypertension history
diabetes mellitus
smoking history
family history of coronary heart disease
cardiac shock were recorded. After being admitted to hospital within 24hpatients′serum in which high-sensitivity c-reactive protein(hsCRP)
fasting blood glucose(FBG)
creatinine(SCr)
triglyceride(TG)
total cholesterol(TC)
low-density lipoprotein cholesterol(LDL-C)
high density lipoprotein cholesterol(HDL-C)levels were detected;after discharge the patient′s survival time were recorded and the influencing factors of the 5years cumulative survival for patients were analyzed. Results By the end of the follow-up the cumulative survival was 70.1%(19cases lost to follow-up and died of other factors
82 cases died of heart vascular disease and 184 cases survived).Single factor Cox proportional hazards analysis results showed that the factors influencing 5years cumulative survival of patients included age
history of diabetes
smoking history
family history of coronary heart disease
complication of left ventricular dysfunction(LVF)
complication of cardiac shock
complication of ventricular fibrillation
hs-CRP≥10.0mg/L
SCr≥120μmol/L(P
<
0.05);Multiariable Cox proportional hazards analysis results showed that the influence factors of 5years cumulative survivalwere age
the hsCRP≥10.0mg/L
SCr≥120μmol/L(P
<
0.05).Comparing the 5years cumulative survival of patients whose hs-CRP
<
10.0mg/L and hs-CRP≥10.0mg/L
the difference was statistically significant(χ
2
=14.076
P
<
0.01);in the comparsion between that of patients whose SCr
<
120μmol/L and SCr≥120μmol/L
the difference was statistically significant(χ
2
=11.548
P
<
0.01);in the comparison between patients aged 61 to 70with patients aged 70 and older
the difference was statistically significant(χ
2
=7.602
19.842;P
<
0.01).Conclusion hs-CRP
SCr levels and age are the main factors influencing the cumulative survival(5years)of patients with ACS and effective measures could be taken to monitor and control the levels of hs-CRP
SCr which is meaningful for improving the prognosis of patients and reduce mortality.
