摘要

目的

探究超声引导下星状神经节阻滞（SGB）对肥胖女性腹腔镜胆囊手术后睡眠障碍（PSD）的影响。

方法

纳入新疆维吾尔自治区喀什地区第二人民医院2024年3月至2025年6月择期行腹腔镜胆囊切除术的女性患者98例，随机分为SGB组和对照组，每组49例。SGB组患者于麻醉诱导前30 min行超声引导下SGB+全麻；对照组患者进行全麻处理。比较2组患者术后当晚实际睡眠时间、深睡眠时间、浅睡眠时间、快速眼动时间、睡眠效率、深睡眠占比、夜间觉醒次数、清醒时间、浅睡眠占比、快速眼动占比、PSD发生率、匹兹堡睡眠质量指数（PSQI）评分、视觉模拟量表（VAS）评分、术后排气时间、术后24 h恶心呕吐发生率。

结果

与对照组相比，SGB组患者术后当晚实际睡眠时间、深睡眠时间、浅睡眠时间、快速眼动时间、睡眠效率、深睡眠占比均显著增加（

0.05），夜间觉醒次数、清醒时间、浅睡眠占比均显著减少（

0.05）；2组患者快速眼动占比比较，差异无统计学意义（

0.05）。2组患者术后1 d、2 d、7 d的PSQI评分较术前1 d均明显升高（

0.05）。与对照组比较，SGB组患者术后1 d、2 d、3 d、5 d、7 d的PSQI评分及术后1 d、2 d的PSD发生率均明显降低（

0.05）。2组患者术后不同时点活动状态下VAS评分比较，差异无统计学意义（

0.05）。SGB组患者术后排气时间短于对照组（

0.05），术后24 h恶心呕吐发生率低于对照组（

0.05）。

结论

超声引导下SGB可改善肥胖女性患者腹腔镜胆囊切除术后的睡眠质量，降低PSD及恶心呕吐发生率，促进术后胃肠功能恢复。

Abstract

Objective

To investigate the effect of ultrasound-guided stellate ganglion block （SGB） on postoperative sleep disorders （PSD） in obese women with laparoscopic cholecystectomy.

Methods

A total of 98 female patients who underwent elective laparoscopic cholecystectomy in the Second People's Hospital of Kashgar Prefecture in Xinjiang Uygur Autonomous Region from March 2024 to June 2025 were enrolled and randomly divided into the SGB group and the control group， with 49 patients in each group. Patients in the SGB group underwent ultrasound-guided SGB 30 minutes before anesthesia induction combined with general anesthesia， while patients in the control group underwent general anesthesia treatment.The actual sleep time，deep sleep time，light sleep time，rapid eye movement time，sleep efficiency， proportion of deep sleep，number of awakenings at night，awake time，proportion of light sleep，proportion of rapid eye movement on the first night after surgery，PSD incidence，pittsburgh sleep quality index （PSQI） score，visual analogue scale （VAS） score，postoperative exhaust time， and incidence of nausea and vomiting within 24 hours after surgery were compared between two groups of patients.

Results

Compared with the control group， the actual sleep time， deep sleep time， light sleep time， rapid eye movement time， sleep efficiency， and the proportion of deep sleep on the first night after surgery of patients in the SGB group were significantly increased （

0.05），the number of awakenings at night，awake time，and the proportion of light sleep were significantly decreased （

0.05）；There was no statistically significant difference in the proportion of rapid eye movement between the two groups of patients （

0.05）. The PSQI scores 1 day， 2 days， and 7 days after surgery of patients in the two groups were significantly increased compared with those 1 day before surgery （

0.05）. Compared with the control group， the PSQI scores 1 day， 2 days， 3 days， 5

days， 7 days after surgery and PSD incidence 1 day， 2 days after surgery of patients in the SGB group were significantly reduced （P

0.05）. There was no statistically significant difference in VAS scores under active condition at different time points after surgery between the two groups of patients （

0.05）. The postoperative exhaust time of patients in the SGB group was shorter than that in the control group （

0.05）， and the incidence of nausea and vomiting within 24 hours after surgery was lower than that in the control group （

0.05）.

Conclusion

Ultrasound-guided SGB can improve sleep quality， reduce the incidence of PSD and nausea and vomiting， and promote the recovery of postoperative gastrointestinal function in obese female patients after laparoscopic cholecystectomy.
