摘要

目的

探讨低领小弧形切口手术治疗对甲状腺癌患者术后美观、并发症的影响。

方法

回顾性分析2022年1月至2024年1月于南京医科大学第三附属医院接受外科治疗的206例甲状腺癌患者的临床资料，其中103例患者接受传统手术治疗（传统手术组），103例患者行低领小弧形切口手术治疗（低领小弧形切口组）。比较2组患者的手术时间、住院时间、切口长度、术中出血量及视觉模拟量表（VAS）评分等围术期指标；术后3个月，观察患者切口愈合情况，并采用温哥华瘢痕量表（VSS）评估患者术后切口的美观度；记录2组患者术后并发症情况及预后结局。

结果

与传统手术组比较，低领小弧形切口组患者手术时间、住院时间及切口长度均更短（

0.05），术中出血量更少（

0.05），VAS评分更低（

0.05）。低领小弧形切口组患者术后3个月肉眼可见瘢痕较浅，而传统手术组患者瘢痕较为明显。低领小弧形切口组患者VSS各维度评分及总分均低于传统手术组（

0.05）。低领小弧形切口组患者术后并发症总发生率显著低于传统手术组（

0.05）。2组患者复发率比较，差异无统计学意义（

0.05）。

结论

低领小弧形切口手术治疗甲状腺癌患者可缩短手术时间与住院时间、减少术中出血量、减轻术后疼痛，降低术后并发症发生率，且术后切口美观度更优。

Abstract

Objective

To investigate the impact of low-collar small arc-shaped incision surgery on postoperative cosmetic outcomes and complications in patients with thyroid cancer.

Methods

A retrospective analysis was conducted on the clinical data of 206 patients with thyroid cancer who underwent surgical treatment in the Third Affiliated Hospital of Nanjing Medical University from January 2022 to January 2024. Among them， 103 patients received conventional thyroidectomy （the conventional surgery group） and 103 patients underwent low-collar small arc-shaped incision surgery （the low-collar small arc-shaped incision group）. Perioperative parameters including operation time， hospital stay， incision length， intraoperative blood loss， and visual analogue scale （VAS） scores were compared between the two groups. At 3 months postoperatively， the wound healing of patients in the two groups was observed， and the cosmetic appearance of the incision of patients in the two groups was assessed by the Vancouver scar scale （VSS）. The postoperative complications and prognostic outcomes of patients in the two groups were recorded.

Results

Compared with the conventional surgery group， the low-collar small arc-shaped incision group had significantly shorter operation time， hospital stay， and incision length （

0.05）， less intraoperative blood loss （

0.05）， and lower VAS score （

0.05）. At 3 months postoperatively， patients in the low-collar small arc-shaped incision grou

p had visibly shallower scars compared with those in the conventional surgery group.The scores of all VSS dimensions and the total VSS score in the low-collar small arc-shaped incision group were lower than those in the conventional surgery group （

0.05）.The total incidence of postoperative complications in the low-collar small arc-shaped incision group was significantly lower than that in the conventional surgery group （

0.05）.There was no statistically significant difference in the recurrence rate between the two groups （

0.05）.

Conclusion

For patients with thyroid cancer， low-collar small arc-shaped incision surgery can shorten operative time and hospital stay， reduce intraoperative blood loss， alleviate postoperative pain， reduce the incidence of postoperative complications， and achieve superior cosmetic outcomes of the surgical incision.
