摘要

目的:分析ICU呼吸机相关性肺炎（Ventilator associated pneumonia

VAP）的病原菌分布及耐药性变迁。方法:对2009年1月-2010年12月本院ICU收治的行有创机械通气治疗>48 h的630例住院患者的痰培养结果进行回顾性调查

分析病原菌分布及耐药性变化。结果:分离出病原菌358株

其中革兰阴性菌64.2%

革兰阳性菌14.8%

真菌20.9%;本组病原菌中前5位依次为鲍曼/溶血不动杆菌（23.5%）、铜绿假单胞菌（15.9%）、金黄色葡萄球菌（13.1%）、白色念珠菌（13.1%）和肺炎克雷伯氏菌（10.3%）;药敏分析结果显示这几类菌株呈多重耐药性。结论:2010年与2009年相比

本院ICU中VAP常见病原菌耐药现象严重

呈上升趋势

提示临床中应及时监测ICU病原菌耐药趋势变化

以指导合理用药。

Abstract

Objective:To analysis of distribution of pathogen and drug resistance vicissitudes for ventilator associated pneumonia in ICU.Methods:Retrospective investigation was used to analyze the pathogen distribution and resistant bacteria changes in 630 patients’ sputum culture resultes with mechanical ventilation over 48 hours in our ICU in Jan.2009 to Dec.2010.Results:358 strain bacteria were isolated

among the total

gram-negative bacteria was 64.2%

gram-positive bacteria was 14.8%

eumycete was 20.9%.The sequence of 5 leading pathogen were bowman/hemolysis motionless coli(23.5%)

pseudomonas aeruginosa(15.9%)

staphylococcus aureus(13.1%)

candida albicans(13.1%) and klebsiella pneumoniae(10.3%);Medicine sensitivity analysis shows that these kinds of a multiple drug resistance.Conclusion:2010 compared with 2009

phenomenon that drug resistance of common pathogens of VAP in the ICU is serious

and increase.Surveillanced the drug resistance trend of pathogen in ICU in time is critical to guide using rational drug.
