摘要

目的分析半乳糖凝集素-3（Galectin-3）、游离三碘甲状腺原氨酸（FT

）/游离甲状腺素（FT

）联合检测在甲状腺结节（TN）良恶性鉴别诊断中的应用价值。方法选取2017年1月—2020年6月商丘市某医院甲状腺乳腺外科收治的139例TN患者为研究对象

术后病理确诊恶性结节47例

良性结节92例

分别将其纳入良性TN组（n=92）、恶性TN组（n=47）。比较2组患者Galectin-3、FT

/FT

绘制受试者工作特征（ROC）曲线分析Galectin-3、FT

/FT

对TN良恶性鉴别的诊断价值。结果恶性TN组患者FT

水平低于良性TN组

Galectin-3表达阳性占比、FT

/FT

均高于良性TN组

差异均有统计学意义（P

0.05）。Galectin-3、FT

/FT

单一用于TN的诊断鉴别时

Galectin-3的曲线下面积（AUC）值显著高于FT

/FT

Galectin-3表达阳性诊断鉴别恶性TN的敏感性、特异性分别为91.49%、89.13%。联合诊断时

诊断敏感性、特异性分别为98.74%、72.83%。结论 Galectin-3单一检测鉴别诊断TN良恶性有较高的效能

并联鉴别诊断TN良恶性可提升敏感性

对TN良恶性的鉴别诊断价值欠佳。

Abstract

Objective To analyze the application of the Galectin-3（Galectin-3） combined with free triiodothyronine（FT

）/free thyroxine（FT

） test in the differential diagnosis of benign and malignant thyroid nodules（TN）.Methods One hundred and thirty-nine patients with TN admitted to the Department of Thyroid Breast and Surgery of a hospital in Shangqiu City from January 2017 to June 2020 were selected as study subjects

of whom 47 subjects with malignant nodules and 92 subjects with benign nodules were diagnosed by postoperative pathology

which were included in the benign TN group（n=92） and malignant TN group（n=47）

respectively.The diagnostic value of Galectin-3 combined with FT

/FT

for the differentiation of benign and malignant TN was analyzed by comparing Galectin-3 and FT

/FT

in the 2 groups and plotting the receiver operating characteristic（ROC） curve.Results The FT3 levels of patients in the malignant TN group were lower than those in the benign TN group

and the percentage of positive Galectin-3 expression and FT

/FT

ratio were higher than those in the benign TN group

and the differences were statistically significant（P

0.05）.When Galectin-3 and FT

/FT

were used for differential diagnosis of TN separately

the AUC of Galectin-3 was significantly higher than that of FT

/FT

（P

0.001）

and the sensitivity and specificity of positive Galectin-3 expression for diagnostic differentiation of malignant TN were 91.49% and 89.13%

respectively.When Galectin-3 and FT

/FT

were used for combined diagnosis

the AUC values of Galectin-3 and FT

/FT

parallel tests were higher than those of tandem tests（P=0.002）

and the diagnostic sensitivity and specificity were 98.74% and 72.83%

respectively.Conclusion Galectin-3 single test has high efficacy in the differential diagnosis of benign and malignant TN

and its combination with FT

/FT

ratio in differential diagnosis can improve the sensitivity but decrease the specificity.FT

/FT

ratio and Galectin-3 tandem FT

/FT

ratio are of poor value in the differential diagnosis of benign and malignant TN.
