Methods: Objective To explore the clinical efficacy of Billroth-Ⅱcombined with enterostomy on patients under-going laparoscopic-assisted gastrectomy for distant gastric cancer. Methods The clinical data of 120 patients who underwent laparoscopic-assisted gastrectomy for distant gastric cancer in a hospital in Liaoning Province from January 2018 to October 2019 were analyzed retrospectively. According to the techniques of digestive tract recon-struction
64 patients were treated with Billroth-Ⅱ combined with enterostomy
which were taken as the com-bined group. Another 56 patients with simple Billroth-Ⅱ were taken as the single group. The perioperative indi-cators
the recovery of gastrointestinal function
postoperative complications and the Visick grade of 6 months af-ter operation were compared between the two groups. Results There was no significant difference in operation time
digestive tract reconstruction time and intraoperative blood loss between the two groups(P>0.05); but the postoperative length of stay in the combined group was significantly shorter than that in the single group(P<0.05). There was no significant difference in bowel sound recovery time
anal exhaust time and liquid diet time between the two groups(P>0.05). There was no significant difference in the incidence of intestinal ob-struction and postoperative infection between the two groups(P>0.05); but the incidence of diarrhea
gastroe-sophageal reflux and gastroparesis in the combined group was lower than that in the single group; and the inci-dence of stomal ulcer in the combined group was higher than that in the single group; the differences were statis-tically significant(P<0.05). Six months after operation
the Visick grade of the combined group was better than that of the single group
and the difference was statistically significant(Z=2.130
P<0.05). Conclusion Compared with simple Billroth-Ⅱ
Billroth Ⅱ combined with enterostomy is more helpful to promote the recov-ery of gastrointestinal function and can reduce the incidence of postoperative complications in patients with gastric cancer
but stomal ulcer caused by anastomotic shunt should be noticed and prevented.
