Objective Toexploretheprognosticvalueofserumprocalcitonin(PCT)combinedwithextravascularlung water index(EVLWI) in patients with acute respiratory distress syndrome(ARDS) caused by severe com-munity-acquired pneumonia(SCAP). Methods A retrospective analysis of the data of 57 SCAP-induced ARDS patients who were hospitalized in the emergency medicine department of a hospital in Huanggang from January 2015 to January 2019 was conducted. The patients were divided into death group(25 cases) and survival group(32 cases) according to their prognosis. The two groups were compaered in terms of white blood cell(WBC) count
C-reactive protein(CRP)
PCT
central venous pressure(CVP)
mean arterial pressure(MAP)
cardiac index(CI)
systemic vascular resistance index(SVRI)
global end diastolic volume index(GEDVI)
EVLWI
oxygenation index(OI) and other indicators on admission. Logistic regression analysis was used to analyze the independent risk factors for death of SCAP-induced ARDS patients
and the receiver operating characteristic(ROC) curve was used to analyze the prognostic value of PCT combined with EVLWI in SCAP-induced ARDS patients. Results The serum PCT level and EVLWI of the patients in the death group on admission were(26.79±7.01)ng/m Land(11.45±2.55)m L/kg
respectively
significantly higher than those of the survival group
which were(7.87±2.72)ng/m Land(7.91±1.37) m L/kg
respectively(all P<0.05). The OI of pa-tients in the death group was(140.51±18.79) mm Hg(1 mm Hg=0.133 k Pa) on admission
which was lower than the(159.14±20.35)mm Hg of the survival group(P<0.05). There was no statistically significant differ-ence in WBC count
CRP
CVP
MAP
CI
SVRI and GEDVI between the two groups on admission(P >0.05). Logistic regression analysis showed that the increase of serum PCT level and EVLWI were independent risk factors for death of SCAP-induced ARDS patients(P<0.05). The sensitivity
specificity and area under the curve(AUC) of PCT combined with EVLWI in the prognosis of SCAP-induced ARDS patients were 91.67%
90.91%
and 0.917
respectively
better than using PCT or EVLWI alone. Conclusion The prognostic value of PCT and EVLWI in SCAP-induced ARDS patients are significantly better than other indicators
and the combina-tion of the two can significantly improve the accuracy of prognostic evaluation.
