Objective To explore the effect individualized community chronic management for patients with chronic diseases and provide scientific evidence for the management of chronic diseases in community.Methods From January 2014 to December 2015 in a community health center of Tianjin 1 000 patients with chronic diseases who had health examination were randomly selected as objects.And the objects were divided into control group（n=500）and observation group（n=500）randomly.The patients of the control group were manged as a whole

while the patients from the observation group were manged individually.After the intervention the aspects of health behaviors

life quality score

self-efficiency score and self-care ability of patients from 2groups′were compared.Results After the intervention the aspects of health behaviors

life quality total and factor score

self-efficiency score and self-care ability of patients from observation group were better than control groups and the difference was significant（P<0.01）.Conclusion As to the community chronic disease management

individualized community chronic management

which is based on the results of heath examinations of patients

could improve the patients′self management efficiency and self-care ability

this help patients maintain health behaviors and improve life quality.
