Objective To investigate the causal relationship between obstructive jaundice（OJ）and renal function impairment and put forward prevention and control measures.Method Relationship between the serum bilirubin and related factors in 409patients with OJ and the renal function impairment was analyzed

and the serum creatinine level before and after surgery was compared.Results Duration of jaundice

the level of serum bilirubin

biliary tract infection and hypotensive state were main factors causing renal function impairment

and the surgery further impaired renal function.Conclusion Hyperbilirubinemia

endotoxemia and change of effective circulating blood volume are direct causes of renal function impairment.Implementing early comprehensive prevention and control is the fundamental measure for protecting renal function

preventing renal impairment and reducing fatality rate.
