摘要

慢性病的社区预防是慢性病防治最有效的手段。本文结合北京大学医院慢性病的管理模式

包括管理人员的组成、管理内容和随访情况

总结了近年来慢性病的管理成效

分析了慢性病管理中的若干问题

并提出了慢性病管理要规范化、系统化、长期化及责任落实到人、加强居民健康教育等相关建议。

Abstract

Community prevention of chronic illness is the most effective way to prevent chronic illness.Combined with the management model of chronic illness of Peking University including the constitution of people

contents of management and the status of follow-ups

this article summarizes the management efficiency of chronic illness in recent years

and it analyzes various problems in the management of chronic illness.The article proposes relevant suggestions as well.For example

the management of chronic illness should be standardized

systematized and long-term;the responsibility should be placed on staff members;and the health education on residents should be enhanced.
