摘要

本文根据我国慢性病管理的现状及高校医院的特点

论述了高校医院开展健康管理的理论与实践。探讨如何开展健康管理和慢性病干预的对策和做法:收集服务对象的个人健康信息;对个人的综合健康及疾病危险性进行评估与定性定量分析

对危险因素采取干预措施

包括生活方式、膳食营养、心理健康等进行个体化指导;定期进行跟踪观察及效果评价

并制定未来的保健计划等。文章还针对存在的问题提出建议。

Abstract

This paper is about the theory and practice of health management at university’s infirmary

according to the status of chronic disease management and characteristics of university infirmary. And it not only discusses the strategies and deeds towards development of health management and intervention of chronic diseases.
