摘要

目的

探讨住院医师规范化培训（简称“住培”）年度业务水平测试结果与结业考核理论考试成绩的关系，及其对结业考核理论考试是否通过的预测作用。

方法

以南通大学第二附属医院2019年至2021年参加住培年度业务水平测试，且2020年至2022年参加住培结业考核的住培医师为研究对象。收集住培医师的性别、学历、人员类型、培训专业、年度业务水平测试成绩、结业考核理论成绩等资料。根据住培医师是否通过结业理论考试将住培医师分为通过组和未通过组。采用SPSS 19.0对数据进行卡方检验、独立样本

检验和二元Logistic回归分析，相关性检测采用Pearson相关系数，敏感性分析以ROC曲线表示。

结果

与通过结业考核理论考试的住培医师相比，未通过考试的本单位住培医师比例明显减少，外单位委培住培医师比例明显增高（

=7.00，

=0.008）。通过组年度业务水平测试全国成绩百分位[（43.46±26.61）%]明显高于未通过组[（23.40±18.71）%]

，差异有统计学意义（

=6.02，

0.001）。年度业务水平测试全国成绩百分位与结业考核理论考试成绩呈低度正相关（

=0.43，

0.05）。住培医师来源为外单位委培和年度业务水平测试全国成绩百分位低是结业考核理论考试不通过的独立危险因素（

=0.020 vs.

0.001）。年度业务水平测试全国成绩百分位预测结业考核理论考试通过的曲线下面积（area under curve ，AUC）是0.73（95%CI=0.65~0.80），具有一定的预测价值，预测的临界值为15.10%。

结论

加强同质化管理和住培专业基地管理的同时，充分利用住培年度业务水平测试结果，对全国成绩百分位低于15.10%的住培医师的专业知识及时进行查漏补缺，可有效提高结业考核理论考试通过率，提升培训质量。

Abstract

Objective

To investigate the correlation between the annual professional proficiency test results and the theoretical examination score of completion assessment in standardized residency training

as well as the value of the annual professional proficiency test results in predicting whether a resident passes the theoretical examination of completion assessment.

Methods

The residents who participated in the annual professional proficiency test of residency training in Affiliated Hospital 2 of Nantong University in 2019-2021 and the completion assessment of residency training in 2020-2022 were selected as subjects

and related data were collected

including sex

education background

personnel type

training specialty

the results of annual professional proficiency test

and the theoretical examination score of completion assessment. According to whether the resident passed the theoretical examination of completion assessment

they were divided into passed group and failed group. SPSS 19.0 was used to perform the chi-square test

the independent samples

-test

and the binary logistic regression analysis; the Pearson correlation coefficient was used for correlation analysis; the sensitivity analysis was represented by ROC curve.

Results

Compared with the residents who passed the theoretical examination of completion assessment

the residents who did not pass the examination had a significant reduction in the proportion of the residents from our hospital and a significant increase in the proportion of the residents commissioned by foreign institutions (

=7.00

=0.008). The passed group had a significantly higher national percentile of annual professional proficiency test score than the failed group (43.46%±26.61% vs. 23.40%±18.71%

=6.02

0.001). The national percentile of annual professional proficiency test score was positively correlated with the theoretical examination score of completion assessment (

=0.43

0.05). The source of residents commissioned by foreign institutions and the low percentile of annual professional proficiency test score were independent risk factors for failing the theoretical examination of completion assessment (

=0.020 and

0.001). The national percentile of annual professional proficiency test score had an area under the ROC curve of 0.73 (95% CI=0.65-0.80) in predicting the outcome of theoretical examination and had a certain predictive value with a cut-off value of 15.10%.

Conclusions

In addition to strengthening homogenization and professional base management for residency training

it is necessary to make full use of the results of annual professional proficiency test in standardized residency training and timely check the professional knowledge of the residents whose a national percentile of <15.10%

so as to effectively improve the pass rate of theoretical examination and the quality of training.
