摘要

目的对比分析术前超声造影与超声内镜在膀胱癌临床分期中的应用价值。方法选取我院收治的膀胱癌患者97例

均行超声造影和超声内镜检查

术后均行病理诊断和分期。以患者病理分期结果为金标准

比较超声造影和超声内镜对膀胱癌临床分期的诊断符合率;绘制受试者工作特征（ROC）曲线

比较两种检查方法在膀胱癌临床分期中的价值。结果病理分期结果显示

97例膀胱癌患者T1、T2、T3、T4期分别为27、23、30、17例

超声造影和超声内镜对T1～T4期的诊断符合率分别为77.78%、86.96%、56.67%、88.24%和74.07%、65.22%、36.67%、17.65%

两种检查方法对T1、T2、T3期的诊断符合率比较差异无统计学意义

超声造影对T4期的诊断符合率高于超声内镜（P<0.05）。ROC曲线分析显示

超声造影诊断T4期膀胱癌的敏感性、特异性、阳性预测值、阴性预测值、曲线下面积分别为87.50%、82.35%、58.33%、95.89%、0.849

均高于超声内镜（67.50%、64.71%、29.73%、90.00%、0.661）

差异均有统计学意义（均P<0.05）。结论术前超声造影与超声内镜均可用于膀胱癌临床分期

但超声造影在T4期膀胱癌患者中的诊断价值更高。

Abstract

Objective To compare the application value of preoperative contrast-enhanced ultrasound（CEUS）and endoscopic ultrasonography（EUS）in clinical staging of bladder cancer.Methods A total of 97 patients with bladder cancer who were admitted to our hospital were selected

they were underwent CEUS and EUS before operation and pathological diagnosis after operation.The results of pathological staging were used as gold standard

the coincidence rate of CEUS and EUS in the clinical staging of bladder cancer was compared.Receiver operating characteristic（ROC）curve was drawn to compare the diagnostic value of two methods for bladder cancer staging.Results Among 97 patients with pathological stage

the number of cases in T1

T4 stage were 27

30 and 17

respectively.The coincidence rate of CEUS in T1～T4 stages were 77.78%

and the diagnostic value of CEUS in patients with T4 stage is higher than that of EUS.
