Effects of Ticagrelor on Serum Uric Acid and Creatinine Levels in the Treatment of Patients with Acute Myocardial Infarction
OBJECTIVE: To explore the changes and possible mechanisms of serum uric acid and serum creatinine levels in patients with acute myocardial infarction during different periods of taking ticagrelor. METHODS: A total of 319 patients with acute myocardial infarction in the hospital who were treated with aspirin and ticagrelor antiplatelet(dual anti-platelet therapy) and underwent emergency percutaneous coronary intervention(PCI) from 2018 to 2019 were selected. Serum uric acid and serum creatinine levels were monitored before dual anti-platelet therapy and after 1 week
1 month
3 months
6 months and 12 months of dual anti-platelet therapy
and after 3 months of ticagrelor(aspirin antiplatelet therapy alone) withdrawal. Through data collection
95 patients who were followed up strictly according to the medical orders were extracted
and changes of serum uric acid and serum creatinine levels were compared. RESULTS: After dual anti-platelet therapy
the serum uric acid and serum creatinine levels of the patients increased
peaked after 1 month of treatment
and then gradually decreased. After 12 months of dual anti-platelet therapy
the serum uric acid and serum creatinine levels tended to be stable
and there was no significant difference compared with those before dual anti-platelet therapy and 3 months after ticagrelor withdrawal(aspirin antiplatelet therapy alone)(P>0.05). The serum creatinine and serum uric acid levels after 1 month
3 months and 6 months of dual anti-platelet therapy were significantly different from those before dual anti-platelet therapy(P<0.05). After 1 week
1 month
3 months
6 months and 12 months of dual anti-platelet therapy
there was no significant difference in serum creatinine levels(P>0.05). After 1 week
1 month
3 months
6 months and 12 months of dual anti-platelet therapy
the serum uric acid levels were significantly different after 1 week and 1 month of dual anti-platelet therapy(P<0.05)
and the other pairwise comparisons showed no significant difference(P>0.05). CONCLUSIONS: Tegretol can cause a significant short-term increase in serum uric acid and serum creatinine levels in patients with acute myocardial infarction to reach a plateau
yet long-term administration gradually decreases and stabilizes. On the one hand
the condition has been improved
and on the other hand
ticagrelor can increase the concentration of adenosine
and increase the serum uric acid and serum creatinine levels
which can reduce the serum uric acid level by increasing the release of NO and inhibiting the activity of xanthine oxidase.
