摘要

目的：探讨自拟温阳通络汤治疗阳虚血瘀证糖尿病周围神经病变的临床效果。方法：根据信封抽签法，将2019年4月至2021年6月该院收治的阳虚血瘀证糖尿病周围神经病变患者120例均分为对照组和研究组。对照组患者接受常规治疗，研究组患者在对照组的基础上接受自拟温阳通络汤治疗。观察两组患者的疗效、中医证候评分、血糖代谢指标水平、神经传导速度、炎症因子水平和不良反应发生情况。结果：研究组患者的总有效率为93.33%(56/60)

较对照组(73.33%

44/60)更高，差异有统计学意义(P<0.05)。两组患者治疗4周后手足麻木、灼热感、感觉异常和肢末时痛等评分，空腹血糖、餐后2 h血糖、白细胞介素(IL)1β、IL-18和肿瘤坏死因子α水平均较治疗前降低，且研究组患者低于对照组，差异均有统计学意义(P<0.05)。两组患者治疗4周后正中神经、右腓神经的运动神经传导速度和感觉神经传导速度较治疗前加快，且研究组患者快于对照组，差异均有统计学意义(P<0.05)。两组患者均未见明显的不良反应。结论：自拟温阳通络汤治疗阳虚血瘀证糖尿病周围神经病变，可改善患者神经传导速度，减轻机体炎症反应，疗效显著。

Abstract

OBJECTIVE: To investigate the clinical effect of self-made Wenyang Tongluo decoction in the treatment of diabetic peripheral neuropathy（DPN） of yang-deficiency and blood-stasis syndrome. METHODS: According to the envelope lottery method

totally 120 patients with DPN yang-deficiency and blood-stasis syndrome admitted into this hospital from Apr. 2019 to Jun. 2021 were equally divided into control group and research group. The control group was given conventional therapy

the research group was given self-made Wenyang Tongluo decoction based on the control group. The efficacy

the motor nerve conduction velocity and sensory nerve conduction velocity of median nerve and right peroneal nerve of both groups were faster than before treatment

and those of the research group were faster than the control group

with statistically significant differences（P<0.05）. No significant adverse drug reactions were observed in both groups. CONCLUSIONS: The self-made Wenyang Tongluo decoction in the treatment of DPN of yang-deficiency and blood-stasis syndrome can improve the nerve conduction velocity and reduce the inflammatory response

with significant efficacy.
