Clinical Analysis on Antibiotic-Associated Diarrhea in Intensive Care Unit Patients
摘要
目的:探讨中国医科大学附属第一医院(以下简称"我院")加强监护病房(intensive care unit
OBJECTIVE: To probe into the risk factors of antibiotic-associated diarrhea( AAD) in intensive care unit( ICU) patients in the First Affiliated Hospital of China Medical University( hereinafter referred to as "our hospital")
so as to provide evidence for formulating effective prevention and control measures. METHODS: Clinical data of 30 ADD patients( ADD group) in ICU( Surgical ICU
Neurological ICU
Respiratory ICU
Cycle ICU and Emergency ICU) in our hospital from Jan. 2018 to Jan. 2019 were retrospectively analyzed
another 120 patients without antibiotic-related diarrhea during the same period were randomly selected as the control group( non-AAD group)
univariate and logistic regression analysis were performed. RESULTS: The incidence of AAD in ICU patients in our hospital was related to the application time( t = 6. 171
P < 0. 05)
category( t = 3. 951
P < 0. 05) of antibiotics and whether used in drug combination; the incidence of AAD in patients with three and above drug combination of antibiotics was obvious higher than that of antibiotics alone
with statistically significant difference( χ2= 4. 327
P < 0. 05); the application time of antibiotics( P = 0. 034)
the application of fluoroquinolones( P =0. 000) and β-lactamase inhibitor( P = 0. 004) were the significant risk factors of AAD in ICU patients.CONCLUSIONS: Strictly grasp the indications for the application of antibiotics
under the precondition of ensuring effective treatment
shorten the time of medication as much as possible
choose antibiotics with low probability of inducing AAD
choose narrow-spectrum antibiotics
avoid unnecessary drug combination and avoid frequent replacement of antibiotic are the important measures to avoid AAD in ICU patients
