摘要

目的:评价医联体模式下苏州市姑苏区某社区卫生服务中心（以下简称"该社区卫生服务中心"）处方点评取得的效果

促进处方点评工作的不断完善

切实提高基层处方质量和指导合理用药。方法:随机抽取2018—2019年该社区卫生服务中心门诊处方2 400张（每月100张）

进行合理性点评;填写《处方点评工作表》

对点评结果进行对比分析。结果:相较于2018年

2019年该社区卫生服务中心门诊处方合理率提高了6%（92%vs. 86%）

抗菌药物使用率降低了5%（12%vs. 17%）

注射剂使用率降低了2%（2%vs. 4%）

处方平均金额降低了5. 26元（114. 77元vs. 120. 03元）;当年所抽查处方中

不规范处方、用药不适宜处方所占比例分别降低了1. 17%（1. 67%vs. 2. 83%）、4. 83%（6. 33%vs. 11. 17%）。结论:医联体模式下该社区卫生服务中心处方点评工作取得了良好效果。全科医师、药师共同参与点评

医院药学专家指导

点评意见反馈以及点评结果与绩效考核关联等干预措施对处方点评的顺利进行起到了重要作用。该社区卫生服务中心将继续推进处方点评工作

提升基层合理用药水平。

Abstract

OBJECTIVE: To evaluate the achieved effect of prescription review in Community Health Service Center of Gusu District（ hereinafter referred to as "this CHSC"） under the medical alliance model

so as to promote the continuous improvement of prescription review

effectively improve the prescription quality and guide rational medication. METHODS: Totally 2 400 outpatient prescriptions were randomly extracted（ 100 prescriptions per month）from this CHSC during 2018-2019

and evaluation on rationality of prescriptions was conducted; filled in the"Prescription Review Worksheet"to compare and analyze the review results. RESULTS: Compared with 2018

the rational rate of outpatient prescription in this CHSC had increased by 6%（ 92% vs. 86%）

the usage rate of antibiotics had decreased by 5%（ 12% vs. 17%）

the usage rate of injections had decreased by 2%（ 2% vs. 4%）

the average amount of money had decreased by 5. 26 yuan（ 114. 77 yuan vs. 120. 03 yuan）; of the extracted prescriptions

the proportions of nonstandard prescriptions and improper medication prescriptions had decreased by1. 17%（ 1. 67% vs. 2. 83%） and 4. 83%（ 6. 33% vs. 11. 17%）

respectively. CONCLUSIONS: The prescription review has achieved good results in this CHSC under the medical alliance model. General practitioners and pharmacists participate in the prescription review together

guidance provided by hospital pharmacy expert

intervening measures like review feedback and review results associated with performance appraisal have played an important role in the processing of prescription review. This community health service center will continue to promote prescription review to improve rational drug application in primary medical care.
