摘要

目的:探讨化浊抗纤保肝汤治疗慢性乙型肝炎肝纤维化的临床效果。方法:选取2018年9月至2019年9月淮安市第四人民医院收治的慢性乙型肝炎肝纤维化患者108例为研究对象

按随机数字表法分为对照组（n=54

采用重组人干扰素α2b治疗）和研究组（n=54

采用化浊抗纤保肝汤+重组人干扰素α2b治疗）

两组患者均治疗3个月。比较两组患者的临床疗效、肝功能指标、肝纤维化指标、组织学评分、免疫功能指标及不良反应发生情况的差异。结果:研究组患者的临床总有效率为88. 89%（48/54）

明显高于对照组的74. 07%（40/54）

差异有统计学意义（P <0. 05）。治疗后

研究组患者天门冬氨酸氨基转移酶、丙氨酸氨基转移酶、总胆红素、层粘连蛋白、透明质酸及Ⅳ型胶原水平明显低于对照组;研究组患者肝纤维化程度、炎症活动度评分明显低于对照组;研究组患者CD8+水平明显低于对照组

而自然杀伤细胞、CD4+及CD4+/CD8+水平明显高于对照组

上述差异均有统计学意义（P <0. 05）。治疗期间

对照组、研究组患者的不良反应发生率分别为14. 81%（8/54）、9. 26%（5/54）

差异无统计学意义（χ2=0. 787

P=0. 375）。结论:慢性乙型肝炎肝纤维化患者采用化浊抗纤保肝汤治疗具有较好的临床疗效

可改善肝功能及纤维化严重程度

降低肝脏炎症反应

提高免疫功能

安全性较高。

Abstract

OBJECTIVE: To probe into the clinical effect of Huazhuo Kangxian Baogan decoction in the treatment of chronic hepatitis B（ CHB） liver fibrosis. METHODS: Totally 108 patients with CHB liver fibrosis admitted to the Fourth People’s Hospital of Huai’an from Sept. 2018 to Sept. 2019 were selected and divided into control group（ 54 cases

treated with recombinant human interferon α2 b） and research group（ 54 cases

given Huazhuo Kangxian Baogan decoction combined with recombinant human interferon α2 b） via random number table

both groups were treated for 3 months. Differences in clinical efficacy

liver function indicator

liver fibrosis indicator

histological score

immune function indicator and incidences of adverse drug reactions were compared between two groups. RESULTS: The clinical total effective rate of research group was 88. 89%（ 48/54）

which was significantly higher than that of the control group（ 74. 07%

laminin

hyaluronic acid and type-Ⅳ collagen levels of research group were significantly lower than those of the control group; the degree of liver fibrosis and score of inflammation activity of research group were significantly lower than those of the control group; the CD8+of research group was significantly lower than that of the control group

while the nature killer cell

CD4+and CD4+/CD8 levels were significantly higher than those of the control group

with statistically significant differences（ P < 0. 05）. During treatment

the incidences of adverse drug reactions of control group and research group were respectively 14. 81%（ 8/54） and 9. 26%（ 5/54）

the difference had no statistical significance（ χ2= 0. 787

P = 0. 375）. CONCLUSIONS: Huazhuo Kangxian Baogan decoction has good clinical efficacy in the treatment of CHB liver fibrosis

which can improve liver function and severity of fibrosis

reduce liver inflammation

promote immune function

with high safety.
