摘要

目的:了解他克莫司的药品不良反应（adverse drug reaction

ADR）及药物相互作用

为该药的临床合理使用提供参考。方法:检索2000年1月至2020年1月中国期刊全文数据库、中国生物医学文献数据库和万方医学网

纳入有关他克莫司致ADR和药物相互作用的病例报道文献

进行统计分析。结果:共检索出他克莫司致ADR文献44篇

涉及69例患者;药物相互作用文献20篇

涉及36例患者;共105例患者符合ADR诊断标准。105例患者中

器官移植患者75例

占71.4%; 3个月内发生ADR的患者共80例

占76.2%;死亡1例。ADR共累及器官和（或）系统116例次

以血液系统为主

共62例次

占53.4%。与他克莫司联合应用的药物主要涉及抗真菌药、钙通道阻滞剂、盐酸小檗胺及五酯胶囊等。结论:他克莫司致ADR的发生与联合用药、患者代谢能力及血药浓度等因素相关

且个体差异较大。因此

临床应高度重视

密切观察用药后患者ADR发生情况并及时处理。

Abstract

OBJECTIVE: To investigate the adverse drug reactions （ADR） and drug interaction of tacrolimus

so as to provide reference for the rational clinical use of drugs. METHODS: CNKI

CBM and Wanfang database were retrieved from Jan. 2000 to Jan. 2020

and case reports on tacrolimus-induced ADR and drug interaction were collected for statistical analysis. RESULTS: A total of 44 articles on ADR induced by tacrolimus were retrieved

including 69 patients. There were 20 literature on drug interaction

including 36 patients. A total of 105 patients met the diagnostic criteria for ADR. Among the 105 patients

75 cases were organ transplant

accounting for 71. 4%; a total of 80 patients （76. 2%） developed ADR within 3 months; there was 1 death. ADR involved organs and （or） system in 116 casetimes

mainly for blood system

totally 62 case-times

accounting for 53. 4%. Drugs used in combination with tacrolimus mainly included antifungal drugs

calcium channel blockers

berbamine hydrochloride and Wu Zhi capsules.CONCLUSIONS: The occurrence of ADR induced by tacrolimus is related to factors such as drug combination

patients’ metabolic capacity and blood drug concentration

and the individual differences are significant. Therefore

clinical attention should be paid to closely observe the occurrence of ADR in patients after administration and timely treatment should be given.
